Print and complete our non-urgent patient referral form and fax to us at 262-650-6064. Please include last visit notes or pertinent medical records. All cataract evaluation requests must include last visit notes.
If referring specifically for oculoplastic, feel free to utilize our non-urgent oculoplastic patient referral form. Please fax to us 262-650-6064.
